Who we are

Alzheimer Scotland is Scotland’s national dementia charity. Our aim is to make sure nobody faces dementia alone. We provide support and information to people with dementia, their carers and families, we campaign for the rights of people with dementia and fund vital dementia research.

What you’ll do

As an Alzheimer Scotland Young Onset Post Diagnostic Support Link Worker (YOPDSLW) you will be part of a National Team to deliver a high quality, person-centred outcome focussed service to people newly diagnosed with young-onset dementia and their families.

As a YOPDSLW, you will use Alzheimer Scotland’s 5 pillar model as a guide to deliver high quality PDS to people with young-onset dementia. The model includes understanding the illness and coming to terms with the diagnosis, peer support, connecting and maintaining links with their community along with planning for future care and decision making to develop an outcome focused plan to support their future hopes, desires and aspirations.

This support will be delivered in a person-centred flexible way at a pace that suits those receiving the service and ensures their voice is heard, along with equipping people living with dementia, and those who care for them, with the information, strategies, connections, resources and plans they need to live as well as possible, preparing for a period of supported self-management.

Supporting our Glasgow locality, the post will be based within Alzheimer Scotland Bridgeton Brain Health and Dementia Resource Centre. You will be an effective team worker and have the skills to build and maintain relationships with colleagues both within Alzheimer Scotland, the NHS and other stakeholders.

Creativity and innovation are encouraged to ensure that we strive to continually improve the way we support people living with dementia and their families.

Skills in using digital platforms are essential as we use a blended approach to support people that involve some virtual meetings and on-line groups.

What you’ll have

The successful candidate will have a flexible approach and able to deal with a number of enquiries and partnerships driven from the community and external partners. You’ll be a fantastic communicator who listens effectively to our service users and provides clear, concise and detailed advice to a varied audience. A positive leadership approach is key to ensuring the running of this service.

Applicants must have a full and valid driving license and vehicle to be able to travel for this role.

Applicants must hold a recognised relevant professional qualification, have a good understanding of dementia and how it affects people and their families along with excellent communication skills, a warm flexible approach, combined with skills in empathy and relationship building.

In addition, you should have skills and knowledge at the Enhanced Level of the Promoting Excellence Framework or be able to evidence you are working towards its completion.
