Role

Phoenix Futures is looking for a dedicated individual to work within the Glasgow Residential Service who will be responsible for co-ordinating and conducting all assessments for admission into the service.  

You will manage all referrals, ensuring assessments are conducted in a timely manner, and manage all admissions into the service with the Admissions Team. 

As the Assessment & Referral Worker you will be the lead worker supporting individuals accessing the residential rehabilitation programme. You will manage all enquiries and referrals made to the service, undertake comprehensive needs assessments for individuals and liaise with the service managers and clinical leads to make placement decisions regarding the suitability.

You will also welcome new residents into the service and ensure all needs of their needs are met upon arrival. Be responsible for data management of referrals in service and report data in line with processes in place. Liaise with commissioners and stakeholders ensuring customer relationship management processes are in place, promoting the service

The role will also supervise volunteers in line with organisational policy to support with preparation work for individuals who have been assessed as suitable for the service. 

This is a full-time role working 37.5 hours per week, Monday to Friday.

About You

  • Relevant experience of working in health and social care, ideally within the field of problematic substance use, mental health or another comparable field
  • Evidence of effective partnership working and the ability to network for the benefit of the client and service
  • Evidence of managing a busy caseload or tasks and excellent organisational skills
  • To be proficient in the use of Microsoft Outlook and Word and be familiar with database management systems
  • Excellent communication skills, both written and verbal
  • Understand complex issues presented by people entering abstinence based residential settings
  • Determined, with a drive to succeed and a willingness to learn
  • Passionate and enthusiastic about making a real difference to the lives of people we support
