  • Maintain accurate records, contribute to case management, and support evaluation.
  • Uphold professional boundaries, confidentiality, and data protection standards.
  • Manage health and safety risks and report concerns appropriately.
  • Attend team meetings and supervision and contribute to ongoing service development.

We’re looking for someone who:

  • Has experience working with people with long-term health conditions or mental health challenges.
  • Is confident working one-to-one and in group settings.
  • Understands trauma-informed and rights-based approaches.
  • Is compassionate, reflective, and committed to promoting choice and independence.
  • Is skilled in communication, building trust, and assessing individual needs.
  • Can manage their workload independently while contributing to a team culture.
  • Is confident using digital tools for communication and record-keeping.
  • Has a strong understanding of data protection, confidentiality, and professional boundaries.

Desirable

  • Lived experience of managing a long-term condition or mental health challenge.
  • Experience facilitating peer support or community wellbeing groups.
