Community Link Worker

Role

RAMH have been developing Social Prescribing in partnership with General Practitioners since October 2015 and we have Link Workers based in East Renfrewshire. The role of this innovative and exciting Social Prescribing development is to facilitate access to community support and opportunities for enhancing self-management skills for people who use services. We are looking for individuals who have some experience and an understanding of working with people who may experience a lack of social connections evidenced by loneliness, mental health problems and isolation, and who may also have a range of social problems. You will need to have excellent assessment skills alongside an understanding of how to recognise and develop assets/strengths for people and enabling approaches when working within local communities and how this can connect with people’s recovery journey. You should be pro-active and experienced in using a variety of social media platforms and applications for communication and as an intervention.

You will be able to work independently using a hybrid model of working from GP practices and/or at home and be very pro-active in finding community resources and helping people access local services and activities to promote their sense of wellbeing. You will have the ability to sustain contact with individuals to assist them to maintain activities. In addition, you should be confident working closely with GPs, practice staff and community agencies. You need to have a high level of IT proficiency, as you are required to record your work daily on a range of IT systems.

You will be required to contribute to a database of local resources, maintain accurate records and actively participate in evaluating, monitoring and reporting this innovative service. A relevant HNC or SVQ III or equivalent is required for this post as is a clear awareness of SSSC Codes of Practice.
